Persons residing within the following school districts are eligible to participate in the Patriots program: Madison East, Madison LaFollette, Monona Grove, and Deforest. A release may be required if you reside outside of any of those school districts.

If you were unable to attend registration please print and complete the registrations forms and mail them to:
